REQUIREMENTS IN APPLYING
FOR BUSINESS PERMIT

All forms downloadable through the links below

For renewal of permanent business permits:

1. Duly accomplished Business Permit Application Form
2. Appendix A and Appendix B of application form
3. Medical Certificate to include employees (to be complied by business establishments classified as eatery, canteen, carinderia, refreshment parlor, restaurant, sari-sari (variety) store, barber shop and beauty parlor)

Note: Please bring copy of latest previous business license or permit

For new applications, include:

1. Barangay Clearance (issued at least 6 months PRIOR to the date of application)
2. Police Clearance (issued at least 6 months PRIOR to the date of application)

Additional Requirements (if needed):

1. Registration from the Bureau of Internal Revenue (BIR)
2. Clearance from the Social Security System (SSS)

For temporary / transient business permits:

1. Duly accomplished Mayor's Permit

Reference for Essential Commodities

1. Rice and Corn
2. Wheat or cassava flour, meat , dairy products, locality manufactured______________
   Preserved, food, sugar, salt and other agricultural, marine, and fresh water products whether in their original state or not.
3. Cooking oil and Cooking Gas
4. Laundry Soap, Detergents and Medicines
5. Equipment and post--- harvest facilities, fertilizer , pesticides, insecticides, herbicides, and other farm inputs.
6. Poultry Feeds and other Animal Feeds
7. School Supplies, and
8. Cement
