1. Barangay Clearance (issued at least 6 months PRIOR to the date of application)
2. Police Clearance (issued at least 6 months PRIOR to the date of application)
3. Judge Clearance (issued at least 6 months PRIOR to the date of application)
4. Mayor's Clearance (issued at least 6 months PRIOR to the date of application)
5. Updated Common Carrier Insurance
6. Certificate of Registration (CR)and Official Receipt (OR) from the Land Transportation Office (LTO) in the name of the applicant
7. Application (form downloadable thru the link below)
8. Inspection Report (form downloadable thru the link below)
9. Certificate of Memmbership from Tricycle Association President
10. Three (3) recently taken colored close-up photographs of the tricycle unit, showing the front view, right-side view and left-side view
